Appointment cancellation of one client may cause other people having difficulty making their appointment on the desired day. To alleviate this situation, we ask our clients to make a deposit at the time of making an appointment. This deposit will be reimbursed on the day of procedure.

 

For those who would like to make an appointment for a single procedure, the deposit of 6,000 yen will be charged. If you would like to make an appointment over the phone, the payment by credit card and the signing this form on the day of the procedure will be requested. You may change your appointment only once if requested 7 business days before the initial schedule or earlier, and the second cancellation costs 6,000 yen. If you cancel within 6 business days of the initial schedule, do not show up, or delay your arrival for 15 minutes or longer, it also costs 6,000 yen. A new appointment requires another 6,000 yen. You may switch to a set of weekday-only 5 procedure course after receiving 1 procedure by paying the difference. You may not do so after receiving more than 1 single procedure.

 

For those of you who would like to make an appointment as part of set procedure (A set of weekday-only 5 procedure course), you may change your appointment up to twice before you complete 5 procedure, if requested 7 business days before the appointment or earlier. The third change of your appointment is counted as one procedure. The change request within 6 business days of the scheduled appointment and the delay of your arrival for 15 minutes or longer requires a new appointment with another 6,000 yen per body part, or is counted as one procedure. If you do not show up at the time of your appointment, it is counted as one procedure. You need to complete the set of 5 procedures within one year from the first procedure day, or the remaining procedure count(s) become(s) invalid. The set of 5 procedures need to be to the same body sites. All procedures need to be done by 5 p.m. on the weekdays.

 

We do not take the cancellation of the weekday-only set procedure after payment, even for the reason listed on the procedure consent form, such as temporal post inflammatory hyperpigmentation.
